Dr. K's clinic

First day at Dr K’s clinic .. He always wears distinct ties and a genuine smile ..

A 25 yr old Thal major on testosterone replacement .. now we discover he has low sperm count .. we speculate and discuss whether he would need fertility treatment .. then he chips in and tells us he has a Thal major girlfriend .. and wants to marry her .. This leads to another bout of counselling where he is told that having a biological child is wrought with risk... then he may not really need fertility preservation, would he ? He looks a little crest-fallen and says “ In the perfect world I wouldn’t have chosen another girl who was suffering from the same malady that I  am .. but this is life .. love happens !“  

Mother of a 6 yr old with multiple pituitary hormone deficiency  breaks down into tears as she tells us she is separated from her husband and fighting for custody .. and that her husband has stopped the treatment, putting her child at risk .. it was heart-wrenching to see the helplessness and anguish on the her face.. tears streaming down ..   
we have to maintain composure as doctors and deal with the medical implications, but sometimes we unconsciously step into the parents shoes and then the enormity and the helplessness really dawns.. this was one of those rare moments.

A 22 year old Engineering student diagnoses himself as Klinefelters after noticing and carefully delineating his symptoms.. !!! ..and then hands the diagnosis over to the experts … !!

A 16 yr old brain tumor survivor, receiving Testosterone injections…His mother describes his erections after the injection , the frequency etc. He goes violently red in the face and kicks her under the table ..

 A 20 year old adolescent trying to act tough while asking whether there’s any treatment available for augmenting his height .. when he reveals he’s one of twins, Dr. K says ”Oh so that’s why he is short” .. He looks really irate” Is that true ! Is my sis the reason I’m short!! He looks aghast .. uh oh .. seemed like there’s going to be a major blame game at home !!

Another mother who was hysterical enough to proclaim that her son wasn’t normal cz he was shorter than the other kids and had “baby fat”.. he got teased a lot at school and she desperately wanted him to grow .. when Sir said he was growing normally on growth charts she stated “ These charts don’t work in real life, doctor. Who made these charts anyways ?”
He says “I made these charts , ma’am”.. I guess that silenced her a little

Lessons from New York

Here I’m describing some revelations I had during my 2 months doing electives in a medical school in New York. P.S.-Please do not be offended by “ We Indians”. I’ve made fun of Americans and Indians equally. Cheers! 1. American movies somehow take place only in New York. 2.No one talks to each other on the plane. You end up missing the loud banter of aunties sitting on side berth and gossiping loudly for the whole train’s benefit. But then they start feeding you on the plane and that somewhat makes up for the lonely feeling :P 3.There are no coolies haggling amongst each other to grab your luggage. Trolleys instead, and one costs 5 $!! (=Rs.250):O:O.. And from here on the dollar to rupee conversion starts in your head… and doesn’t end till you’re back in India. 4.The taxis that we take so much for granted in our country are a luxury in US.. and you’ve to tip the driver just for driving the damned taxi.. hey isn’t that what we’re paying him for in the first place ?...

Overwhelmed

            I was in my alma mater today .. and incidentally got talking with an MBBS student .. she was awed when she heard I was a past MBBS student from the same college and now MD Pediatrics with a Fellowship in Pediatric Endocrinology.. somehow I felt undeserving of that emotion ..  Am I a big deal in the medical world ? What has it really translated into ?  I have all these years of training and labour and sweat .. has it shaped me better, have I evolved or have I also embibed cynicism and a touch of bitterness on the way ? She went on to ask me what had kept me motivated to achieve what I have .. and I muttered something about it not being as great as it appears. . she was a little disbelieving .. I told her it's been a long and arduous journey .. and Im still to see the light at the end of the tunnel. She opened up about her problems then .. being holed up in a roomful of 10 in a strange city; facing sleep...
